This service is used by students living in the Residence Hall in order to request services including: Bed Adjustments, Plumbing Issues, Electrical Issues, Maintenance Issues, Custodial Issues, and other Facilities Related incidents that may occur.

This Service Request form is to be completed by Daytona State College Residence Hall residents only. Residents can use this form to request facilities related repairs or other related requests.

Be sure to complete the Service Request form fully, missing details may result in delays in addressing your requests.
